American Traveler is seeking a Home Health Physical Therapist with a valid CA PT License and at least 1 year of Home Health PT experience.

Details

• Home Health setting with visits made in patients' homes
• Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
• Caseload of approximately 4-5 patients per day, based on 5 weighted visits per day
• 13-week assignment length

Requirements

• Current CA PT License required (license required if applicable)
• Minimum 1 year of experience as a Home Health Physical Therapist required
• Valid driver's license and auto insurance required
• BLS certification preferred
• Experience with home health documentation including OASIS, Plan of Care, and RUG Levels preferred
• Skilled in orthopedic, neurologic, and geriatric patient care preferred

Additional Information

• Responsibilities include patient assessment, home exercise program development, and patient/family education
• Treats a variety of conditions including orthopedic injuries, neurologic disorders, and post-surgical rehabilitation
• Utilizes home health equipment such as gait belts, walkers, Hoyer lifts, and specialty beds as needed
• Maximum RTO of 7 days per contract, with only 1 holiday request approved per contract
• Holiday coverage includes New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day
• Accepts local candidates within a 50-mile radius at the local rate
• Returning staff or travelers must have a separation period before reapplying
