Description

American Traveler is seeking a Physical Therapist with Home Health experience for a 13-week travel contract serving a geriatric patient population across a defined territory in the East Bay area of California.

Details

• Home health setting serving a geriatric patient population
• Territory covers the Far East region, approximately from Discovery Bay to Pittsburg, within a 60-mile radius of the hub/office
• Average caseload of 5–7 patients per day
• Day shift schedule, 5x8-hour shifts (8:00–17:00), Monday–Friday
• No on-call or weekend requirements
• EMR: HCHB (experience required); OASIS experience preferred
• iPad provided for documentation; personal cell phone not required
• Mileage reimbursed at the IRS standard rate

Requirements

• Current PT license required if applicable
• Minimum 1 year of recent home health physical therapy experience
• Current BLS certification required
• Proficiency with HCHB EMR required
• OASIS experience preferred
• Must be flexible with territory coverage
• 2 professional supervisor references from within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration
