Description

American Traveler is seeking a Home Health Physical Therapist with 1+ year of certified home care experience and Massachusetts licensure to provide in-home care across the North Shore region.

Details

• Home health setting providing physical therapy services to patients in their place of residence
• Serves North Region including Lynn, Nahant, and Saugus
• Patient population ranges from birth to over 100 years of age with physical impairments, functional limitations, or disabilities
• Requires travel to patient homes as assigned
• Day shift schedule from 8:00 AM to 4:30 PM
• Weekend rotation once every 6 weeks
• 13-week assignment length
• Uses Epic for charting

Requirements

• Active Massachusetts Physical Therapist license required if applicable
• Minimum 1 year of certified home health care experience required
• Minimum 1 year of case management experience required
• Experience must be specifically within a certified home health agency; hospice, private care, or standalone case management experience does not qualify
• AHA certifications only
• Flu vaccine required with no religious or medical exemptions allowed
• Ability to travel to patient homes as part of assigned caseload

Additional Information

• Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate care
• Maintain thorough and timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice
• Local candidates accepted with permanent residence within 60 miles of Boston, though rate will be 5% less than standard rate
• Public parking available at Boston locations with daily rates ranging from $17-$22; no validated parking provided
