Description

American Traveler is seeking a Home Health Physical Therapist with a minimum of 1 year of recent experience, OASIS proficiency, and HCHB EMR experience for a 13-week assignment.

Details

• Home health setting serving a geriatric patient population
• Average caseload of 5–7 patients per day (to be confirmed during interview)
• Territory covers up to a 60-mile radius from the hub/office; flexibility with territory is expected
• Day shift schedule, 5x8-hour shifts (8:00–17:00)
• On-call rotation required approximately every 3rd to 4th weekend
• EMR: HCHB (Homecare Homebase) experience required
• iPad provided for documentation; personal cell phone not provided
• Mileage reimbursed at the IRS standard rate

Requirements

• Minimum 1 year of recent home health physical therapy experience
• OASIS experience required for Start of Care (SOC) visits
• Proficiency in HCHB (Homecare Homebase) required
• Current BLS certification required
• Active driver's license and personal transportation required
• PT license in hand required for consideration (license required if applicable)
• RTO must be submitted at the time of application

Additional Information

• Responsibilities include physical therapy evaluation and treatment of geriatric home health patients with timely documentation and communication required
• Orientation is held on Mondays; required modules are completed during orientation
• Scrubs of any color are acceptable; jeans are not permitted
• Local candidates are accepted at the travel rate
• First-time travelers are accepted
• Travel pairs are accepted; however, each traveler will be assigned their own territory and must have their own transportation
• Former permanent employees must have been separated for a minimum of 6 months and receive HR approval before returning as a traveler
