American Traveler is seeking a Physical Therapist Assistant with a minimum of 2 years of experience for a hospice home health position working Monday through Friday in the Southwest region.

Details

• Hospice home health setting serving patients in the Southwest region
• Provides rehabilitative and maintenance physical therapy services to a hospice patient population
• Responsibilities include restorative and maintenance therapeutic techniques as well as patient and caregiver education
• Schedule is Monday through Friday, 8:00 AM to 4:00 PM
• 13-week contract

Requirements

• State PTA license required upon acceptance — license required if applicable
• Minimum 2 years of experience as a PTA
• Familiarity with HHCAHPS standards preferred
• Ability to work collaboratively within a multidisciplinary care team

Additional Information

• Carry out individualized treatment plans under the direct supervision of a Physical Therapist in accordance with APTA guidelines and federal and state regulations
• Assist in developing plans of care aimed at promoting optimal functional independence and safety for hospice patients
• Coordinate patient care with physicians, nurses, caregivers, PT team members, and other rehab services
• Monitor and report changes in patient condition to the case manager in a timely manner
• Maintain compliant clinical documentation reflecting care provided and outcomes achieved
• Stay current on HHCAHPS standards and integrate them into visit protocols
