• Outpatient PT setting within a small community hospital in Perry, OK
• Day shift schedule, 5x8-hour shifts (8:00 AM - 5:00 PM)
• 13-week contract
• Uses Quick EMR for documentation

Requirements

• Minimum 2 years of physical therapy experience required
• Bachelor of Science degree from an accredited physical therapy program required
• Current Oklahoma PT license required (license required if applicable)
• BLS certification required
• Two professional references required for consideration
• PDF copies of driver's license and Social Security card required for consideration
