Description

American Traveler is seeking a Physical Therapist Assistant with a current California license and at least 2 years of recent home health experience for a daytime position in the San Jose and South Bay area.

Details

• Work in a home health setting serving the San Jose, South Bay, and Santa Clara County regions
• Day shifts, 5x8 hours, from 8:00 a.m. to 5:00 p.m.
• Patients are seen in their homes as part of an interdisciplinary rehabilitation team
• Care provided includes a range of home health-associated rehabilitative services
• Uses Epic for documentation and charting

Requirements

• Active California PTA license required
• PTA certification and current BLS required
• Minimum 2 years of recent experience in a home health setting
• Experience with Epic EMR required
• License required if applicable

Additional Information

• Must be available to complete lab work within 3-5 days of accepting contract
• Only one holiday off will be approved during the contract period
• Return staff must have been gone for at least one year to be eligible
• 50-mile radius rule applies for candidate eligibility
• Local candidates are accepted
